Enroll at PS 58X Community School today. Follow the steps on the flyer or call the school at (718) 583-6866 to secure your spot!

 

You need the following documents to enroll your child:

1. Proof of student age (example: Birth Certificate, passport, etc.)

2. 2 documents for proof of address (example: Lease, utilities bill addressed to your name, etc.)

3. Your child's latest report card/transcript;

4. State identification document (example: driver's license, passport, etc.)

5. Copy of current immunization records (see this page for the most updated list of requirements: https://www.health.ny.gov/prevention/immunization/schools/school_vaccines/)
